       To start the editorial processing of your article in the Moldovan Medical Journal (MMJ), we kindly ask the corresponding author to fill in, sign, scan and send this form together with the article to editor@moldmedjournal.md.  With this the corresponding author certifies that he personally and co-authors accept the responsibility for the content of this article.

       The signature will evidence the mutual understanding between the author(s) and the MMJ on the rights and responsibilities of both parties in the process of article evaluation and its possible publication in the MMJ.

       The authors are kindly requested to read our web site www.moldmedjournal.md and strictly follow the directions of the Publication Ethics and Malpractice Statement.

       1. Authorship of submitted article
       The MMJ tends to be a member of the International Committee of Medical Journal Editors and subscribes to its Uniform Requirements for Manuscripts Submitted to Biomedical Journals, including its authorship requirements.
       An author (co-author) of a scientific article is considered to be someone who has made substantive contribution to the submitted work.
       The MMJ wants authors to make sure that all authors included in the manuscript fulfill the criteria of authorship, as well as that there is no one else who should be the author of the submitted manuscript but has not been included as an author.
       If you think someone has been omitted from or that someone should not be in the manuscript by-line, please let us know in a separate letter.
       If, in your opinion, there are researches that are included in the by-line but do not deserve to be authors, please let us know in a separate letter.

       2. License to publish, copyright information, transfer of copyright agreement
       The listed authors warrant that they are the authors and sole owners of the submitted manuscript.  The authors also warrant that: the work is original; it has not been previously published in print or electronic version and is not under consideration by another publisher or electronic medium; it has not been previously transferred, assigned, or otherwise encumbered; the authors have full power to grant such rights.
       With respect to the results of this work, the manuscript of this or substantially similar content will not be submitted to any other journal until the review process in the MMJ has been officially completed (acceptance or rejection of the manuscript).
       The authors will comply with the requests of the MMJ editors and reviewers to improve the manuscript for publication.
       The unavoidable disagreements will be submitted in a written form; the authors are aware that the disagreement(s) with the MMJ’s requests may result in the rejection of the manuscript.
       The authors hereby grant to the MMJ the right to edit, revise, abridge, and condense the manuscript.  If the manuscript is accepted for publication in the MMJ, the authors hereby authorize the MMJ to publish their work in the journal.  The authors permit the MMJ to allow third parties to copy any part of the journal without asking for permission, provided that the reference to the source is given and that this is not done for commercial purposes.
       When the article is accepted for publication, the authors hereby agree to transfer to the MMJ all rights, including those pertaining to electronic forms and transmissions, under existing copyright laws, except for the following, which the author specifically retains: the right to make further copies of all or part of the published article for use in classroom teaching; the right to reuse all or part of this material in a review or in a textbook of which he is the author; the right to make copies of the published work for internal distribution within the institution that employs the author.  The journal allows the authors to hold the copyright without restrictions.
       For papers with more than one author the corresponding author agrees and is allowed by co-authors to make decisions regarding prepublication release of information in the paper to the media, government agencies or both.

       3. Conflict of Interest
       The potential conflict of interests should be acknowledged by all the authors and editorial reviewers.  If such a conflict is recognized, the reviewer is excluded from the review process and another reviewer is assigned.  Please let us know about it in a separate letter.

       4. Patients, Animals and Study Participants
       The papers describing a research, involving animal or human subjects, should state in a separate letter that the rules of working with animals have been observed and the official consent has been obtained from the patients, and it has been approved by the designated board of the institution involved.

       5. Editorial Research
       We are keen to better understand and improve editorial conduct, decision making, and issues related to peer review. Therefore, we occasionally take part in or conduct editorial research and your submitted manuscript might be used in such research.  If you do not want your manuscript entered into such a study, please let us know about it in a separate letter.
       We perform article screening for plagiarism.  For more details please read chapter 2.3, 2.4, 3.6 of the Publication Ethics and Malpractice Statement.

       6. Herewith, I confirm that I am the owner (or authorized to act on behalf of the co-authors) of the materials used in this article, and thus I render the permission to use it to the Moldovan Medical Journal.

       7. Herewith, I confirm that I am aware of the rules of submission, technical requirements and peer review process in the Moldovan Medical Journal.
